If an annual meeting is not held in a physical location and voting is conducted by ballot, either by regular mail or online at the ACE of Norwood website, I certify that the above address is my accurate mailing and/or email address and ballots may be received by me at that address. I understand if I do not provide a correct mailing address, my submission will constitute a waiver of notice of any meeting or ballot that I may be entitled to participate in.

MEMBERSHIP / DUES SCHEDULE
The following details the current membership dues and benefits structure. Please select your choice of membership level. To mail your membership dues, check "Check mailed to" option and mail your check, made out to ACE of Norwood. The PayPal option is a Subscription, and will automatically be renewed each year. To pay through PayPal, click on the appropriate button at the desired membership level. You will be taken to a new window to complete the transaction. Be sure to have the "Paid via PayPal" option checked below when registering through PayPal.

Sponsors	$5,000 per year	Above benefits & a year's tickets to ACE events
Patrons	$25,000 per year	Above benefits & 5 free ACE of Norwood classes (materials not included) per year	n/a**
